Medical Director
Flavia L. Thomas, D.O.
I am a Board-Certified Family Medicine physician with an extensive 23-year track record of dedicated patient care. I have expanded my skills through various specialized services with certifications as a medical examiner for the FMCSA, an immigration civil surgeon, a prescriber of medical marijuana, holder of an X-waiver for Suboxone prescription, and a clinical research principal investigator. I am a dedicated educator, simplifying the intricacies of medicine for my patients. Professional yet approachable, I traded in the traditional white coat keeping things down-to-earth and humble. My practice is a blend of expertise and experience with genuine compassion. I always go the extra mile for my patients. My commitment extends beyond medical expertise—I am devoted to improving healthcare disparities and promoting the well-being of individuals and communities. I am a physician who values connection and prioritizes your well-being beyond the confines of clinical walls. Lead Counselor
Brenda Riley, LCDC
I hold a bachelor’s degree in psychology from the University of Houston. I have several certifications including, Parenting Education, Anit-Theft, Moral Recognition Therapy, Anger Management just to name a few. My career in counseling began as a Counselor for a Non-Profit agency assisting students, families, schools, staff, and communities in multiple ISD’s in Texas for 16 years. My compassion and drive for helping people and the lives of others around them continued into my own practice of 10 years where I facilitate court-ordered classes for residents of Texas working with several counties across the State of Texas with a focus on Domestic Violence/Battering Intervention and Prevention Program known as BIPP. I earned a full State of Texas accreditation to facilitate these sessions. I later became a Licensed Chemical Dependency Counselor (LCDC) assisting those affected by the disease of addiction and their loved ones. In this capacity the focus is client centered, assisting clients in understanding the underlying cause of addiction and helping clients learn coping strategies, change behaviors, and recognizing their unique triggers. I am a wife of 35 years, a mother, and joyful grandmother. In my spare time I enjoy spending time with my family and friends, attending church, dancing, socializing and being a super-fan of Prince. I am grateful for the opportunity to guide and support clients on their journey to sobriety.

Yasmeen Mohammed, LVN
Louisiana native with a heart as warm as gumbo, caring wife, multitasking mom, and passionate healthcare professional. With over 10 years of diverse nursing experience, and a Bachelor’s degree in Health Service Administration, I’ve had the opportunity to create and manage health programs on the state and federal level. This experience has truly empowered me to make a significant impact in healthcare access for the population around me. My unique perspective and diverse work history will help to deliver a quality and constructive experience while under our care. When I’m not saving lives and ensuring top-notch patient care, you can find me indulging in my passions for baking delectable treats or exploring the latest trends in makeup and skincare.
